Case Management Coordinate needed services for patients across a range of service providers and settings. Case managers may assist in obtaining services such as medical and mental health care; financial assistance; legal assistance; practical and emotional support; and housing assistance.
Educational Services Providing education to the HIV community, including general information about HIV/AIDS, preventive training for those at risk, community outreach and volunteer programs.
Financial Assistance Providing financial assistance for individuals affected by HIV/AIDS.
Food Pantry / Housing / Transportation Offering services which may include housing assistance, food banks and transportation to medical/social services appointments.
Legal Services Providing legal services to individuals affected by HIV/AIDS.
Testing & Counseling Offering HIV testing and counseling services to individuals, couples and families living with, or affected by, HIV/AIDS; services may include support groups and peer counseling.
